Community verification and sources
Before a community can appear in this directory, its license, required exclusion checks and permission for its business profile must pass a separate review. Listings and claims are free. Sponsorship and paid services cannot change a badge or result order.
What the badges cover
- License-Verified
- A current matching care license, identified operator and administrator, required exclusion checks for the identified subjects, service fit and permission for the exact public profile. Each licensed care type is reviewed separately.
- Verified
- The baseline checks plus reviewed ownership, administrator qualifications and the latest authenticated inspection summary. Any required correction acceptance and completion must be evidenced separately.
Caregiver Assist checks licensing and required exclusion sources. These checks do not evaluate the quality of care or guarantee availability. Visit the community, review its current inspection information and confirm services and costs before deciding.
Official sources for the initial Pennsylvania scope
- PA DHS Human Services Provider Directory: personal care home and assisted living license records, capacity and source status. Inspection history and certificate references require separate review.
- PA Medicheck: Pennsylvania precluded-provider checks.
- HHS OIG List of Excluded Individuals and Entities: the federal health-care exclusion source. A name match needs identity resolution; it is not a confirmed exclusion by itself.
- PA DHS personal care home and assisted living licensing: licensing resources and inspection guidance.
The initial scope covers personal care homes and assisted living residences in Bucks, Chester, Delaware, Montgomery and Philadelphia counties. Nursing homes, New Jersey and Delaware communities are later phases and are not covered by the current PA publication workflow.
Dates, changes and missing information
A listing shows its review and source-observation dates. Expired evidence, unresolved changes or unavailable required checks hold publication. A missing search result or source outage requires investigation and does not establish that a community is unlicensed, excluded or unsafe. Reinstatement requires fresh independent review.
Compare the care you need
Personal care homes and assisted living residences keep their licensed categories. An officially supported special-care designation is separate from facility-reported memory support. Licensed capacity is not a vacancy count.
Services, languages, prices and payment terms are facility-reported. Confirm what the price includes, room and board, any separate care charges and payment eligibility directly with the community.
Representatives and corrections
Facility representatives can request a free claim. An individual representative must establish authority for the specific license. A claim or purchase does not award a badge. Corrections and conflicting claims receive human review.
Use business information only. Do not submit resident records, care-needs narratives or identifying resident photographs.
